Is it possible to claim treatment on Medicare?
Yes. This gives you 5 visits per calendar year under the chronic disease management plan (CDM Plan), formerly known as the enhanced primary care plan (EPC Plan).
How do I get this CDM plan?
If you are eligible for the CDM plan and 5 visits covered by Medicare rebate you must be referred by your general practitioner (GP). To be eligible you must have a chronic medical condition that has been present for at least six months or have a complex medical condition.
These are some examples of chronic medical conditions:
- Lower back pain
- Neck pain
- Arthritis
- Asthma

What should I bring to my appointment?
It will help if you can bring the referral letter from your GP and Medicare card.
Can I have more than 5 visits per year?
Its only possible to receive 5 visits. This resets each calendar year, so if your GP is to refer you again, then you will receive another 5 visits.
What if I need more treatment?
Once your 5 Medicare visits have finished you can start using your health fund on our HICAPS machine so you can claim instantly with only the gap to pay.
